How Your FQHC can attain your Hypertension Control Program Goals

Cardiovascular disease accounts for 1-in-7 US health care dollars spent - almost $1 billion per day - and accounts for 800 000 deaths every year, representing almost a third of US deaths. Hypertension is a major risk factor for cardiovascular disease.
New guidelines for blood pressure management recommend using health information technology to identify patients with elevated blood pressure who are not diagnosed with hypertension. To address this national crisis, the Health Resource and Services Administration (HRSA) recently awarded nearly $90 million in supplemental funds to fund this initiative in 496 Federally Qualified Health Centers (FQHC) participating in the National Hypertension Control Initiative (NHCI-HC).
The NHCI-HC aims to use remote patient monitoring, Bluetooth or wireless-enabled SMBP for patients with hypertension, and to use their data to form future hypertension treatment plans. In the U.S., of the 4.8 million adult patients who are diagnosed with hypertension, 57.7% identify as a racial and/or ethnic minority.
The primary goal of the NHCI-HC is to increase controlled hypertension among this population by leveraging FQHCs to drive improved care delivery and patient monitoring. Over a 3-year period, funding must be used for in-scope services and sites that will expand access to hypertension diagnosis and control. Funding will support a wide-range of diagnosis and testing related in-scope activities that may change as needs evolve within your community.

MDRP, will provide all non-clinical aspects of the program, letting the FQHC staff focus on the clinical aspects of improving patient outcomes. We will also provide all the training and support for both the Health Center staff and patients. When patients’ remote BP readings are taken, they are made available in real-time so clinicians and case managers can act on them if necessary. Clinicians can also use our Remote Care platform to manage Chronic Care Management (CCM), Telehealth, Transitional Care Management (TCM) and Annual Wellness Visits (AWV) to improve the financial outcomes of the FQHC.
